About the Role

To provide community-based mentoring to people 11 years old +. You will work with a caseload of clients for a limited time, which could be a term or two or longer.  Your support will be predominantly on a 1-1 basis and your support will be split between the Community (Surrey and borders) and our Hub in St Johns, Woking.

Responsibilities:
·       To find meaningful community-based opportunities

·       To identify person centered goals and aspirations

·       Liaise with parents, carers, SEND case officers, Care practitioners and other third-party professionals

·       Driving or travel training clients to activities, for example place of work, social groups or therapy sessions

·       Job Coaching clients in a variety work related opportunities

·       Visiting clients and family members in their own homes

·       Lone working, managing your own time and caseload

·       Attending reviews to provide feedback on support provided

·       To complete personal profiles, risk assessments and other documentation to always ensure client safety while in the community

·       Advocate for clients in work and community settings

·       Encourage clients who are anxious by working at their pace through action plans

·       Support clients to engage with activities/sessions/workshops at our Hub in Woking

·       To work flexibly within the business including covering for other staff sickness or holidays both in the community mentoring and at the hub

·       Keeping up to date records and paperwork

·       Keep up to date on mandatory training as well as other recommended training

Essential:
• Must have a driving license and use of a car
• Minimum 5 GCSE or equivalent
• Laptop with Microsoft for record keeping and other admin tasks
• Ability to write professional reports and records
• This is currently a term time only contract, with the view for additional hours during the half term and holidays

Training
• Will be provided, please see the list of mandatory training in the induction pack that EDGE requires you to undertake.
